What is upper crossed syndrome?

Upper Crossed Syndrome (UCS) is a muscle imbalance pattern in the upper body, commonly caused by poor posture, repetitive movements, and/or a combination of tight and weak muscles. The term itself describes the tension and weakness that occurs in the upper body, when looked at from the side it creates a cross due to the line of tension and line of weakness.

The muscles commonly involved along the tight/overactive line are:

  • Upper trapezius
  • Levator scapulae
  • Pectoralis major and minor

The muscles commonly involved along the weak/underactive line are:

  • Deep cervical flexors
  • Rhomboids
  • Lower trapezius
  • Serratus anterior

What impact does this have on your body:

  • Rounded shoulder and hunched back – Due to the muscle imbalance between the tightness through your pectoral muscles and weakness in the upper back muscles the shoulders are pulled forward
  • Forward head carriage – Again due to the muscle imbalance, this time between the tight upper trapezius and levator scapulae, and weakness through the anterior cervical muscles, your head is pushed forward
  • Neck and shoulder pain – The tension in the neck and shoulders can cause aching and discomfort and in some circumstances lead to numbness, tingling and even headaches

Causes:

  • Poor posture
  • Repetitive motion
  • Muscle weakness/imbalance

Treatment:

  • Postural correction: Mindful adjustments to sitting, standing, and moving positions.
  • Strengthening exercises: Focus on strengthening weak muscles like the rhomboids, lower trapezius, and deep cervical flexors.
  • Stretching: Stretch tight muscles like the chest, upper traps, and levator scapulae.
  • Manual therapy: Techniques like massage or physical therapy to reduce tightness and realign muscles.
